Choice Home Care is a Medicare certified, Minnesota Class A licensed home
care agency whose mission is to provide services that promote, maintain
or return you to optimal health.
Choice Home Care offers services throughout Southeastern Minnesota.

The Medication Options Program (MOP) is an integrated, community based program serving adults with a serious and persistent mental illness who have a history of non-compliance with their psychotropic medications or are unable to maintain independent living because of their disability. Developed by Olmsted County Public Health in 1986, it is now a collaborative effort of Olmsted County Social Services and four (4) home health care agencies in Olmsted County, including Choice Home Care. Choice Home Care has provided Medication Options program services since 2002.
The goal of Choice Home Care is to promote health and maintain or restore clients to optimal health so that people can live independently in their homes for as long as possible. Providing Medication Options program services fits well within our mission, as MOP services are integral to the promotion of good mental health and independence of the clients we serve.
Medication Options program nurses provide medication set-up, delivery and administration, symptom and side effect monitoring and physical and mental health assessment and follow-up. Physical health assessment is an important part of program services because many Medication Options clients have co-occurring disorders or disease processes -- some of which can be chronic conditions – such as substance abuse, diabetes, chronic obstructive pulmonary disease, congestive heart failure, obesity or hypertension. Nurses are a key part of program services because of their knowledge of disease processes and health assessment skills.
Individuals are referred to the Medication Options Program by Olmsted County Social Services Adult Mental Health social workers. The frequency of the visits is determined through collaboration with the client, his/her social worker, the assessing registered nurse, and the client’s psychiatrist. This schedule may vary from 2x/day to 1x/week according to the client’s needs.
