About Nursing Home Relocation
SEMCIL provides nursing home relocation coordination services to persons
with a disability currently living in nursing homes or other institutional
settings. Relocation services are important to the work that Centers
for Independent Living do because this service assists consumers who
wish to leave a facility or nursing home and move back into the community.
Staff can help consumers make decisions about the supports he or she
will need outside the institution and assist the individual to access
the services and supports needed such as locating housing, setting up
home and community based services and making sure that the transition
from NH/residential facility to community is successful.
