INDEPENDENT LIVING SERVICES

SEMCIL's Independent Living Skills Program works in partnership with consumers and families to promote independence with daily living activities. Services are provided to all persons with disabilities. SEMCIL has trained personnel experienced in providing skills training specifically designed for persons with disabilities. Staff works with clients on goals identified by the individual. Length of services depends on goal completion and progress.

Our Transition Services Program provides young adults ages 14 - 25 with the skills training necessary to make responsible decisions toward achieving an independent lifestyle. A comprehensive independent living skills assessment is available for youths and adults with disabilities. Daily living skills are assessed through a "hands-on" evaluation.

SEMCIL's Information and Referral Services provide comprehensive, up-to-date information about disability related resources and independent living issues. A directory of agencies that serve persons with disabilities is one of several valuable resources available to consumers and families.

Experienced and knowledgeable Peer Mentors act as role models and assist individuals and families to cope with and manage changes. SEMCIL sponsors several Peer Support Groups that offer educational speakers and opportunities for sharing and support. SEMCIL works cooperatively with the Mayo Clinic Rehabilitation Unit to assure that quality peer support services are available to person with disabilities. SEMCIL also provides information and support to family members. Workshops, and support opportunities are offered to family and others within the individuals support system.

Advocacy Services. SEMCIL serves as the expert with interpreting and assuring equal access for all persons in Southeastern Minnesota through the Americans with Disabilities Act (ADA). Staff assist consumers and families to locate appropriate sources to address employment, transportation, and access-related concerns. More than 30 school districts and over 275 businesses in Southeastern Minnesota have sought SEMCIL's expert advice about issues related to compliance since ADA was enacted. SEMCIL assists individuals who feel they have been discriminated against because of their disability. Individuals may file a grievance with the Department of Justice and staff can offer support with the steps involved with this process.

SEMCIL's Housing/Home Modification/RAMPS program offers information concerning affordable, accessible housing options. SEMCIL has qualified individuals conduct home modifications assessments to identify needed changes to provide a safe independent environment. The center can assist with researching financial options available to assist with modifications.

Disability Awareness Services provide a variety of materials and training programs to increase public awareness and sensitivity of persons with disabilities. This program can be tailored to meet individual, organization or group needs. SEMCIL has developed a disability awareness learning kit, coloring book and speakers bureau that can be accessed by area school districts and community agencies.

Library/Computer Lab. This program is designed to inform the public of services that can help them or someone they know live more independently. The library has an assortment of books, magazines, brochures and videos that relate to disability issues. The library has expanded to include a computer lab. Persons with disabilities are eligible to sign up for classes and sign up for lab time to enhance their knowledge of resources and technology.

PERSONAL CARE ASSISTANCE SERVICES

SEMCIL's Personal Care Assistance Program is an approved Minnesota Medical Assistance provider. The program incorporates 15 years of knowledge, sensitivity, and experience with individual needs and independent lifestyles.

Persons needing assistance with bathing, dressing, grooming, and other personal care needs can select a Personal Care Assistant from SEMCIL's Referral Registry. Personal Care Assistants are matched with consumers based on availability and consumer choice. All Personal Care services are delivered in a manner consistent with a recipients desire to live independently in the community.

Personal Care Assistant candidates participate in SEMCIL's Personal Care Assistance Training Program. Participants receive classroom and clinical training prior to placement with consumers. Registered Nurses meet with consumers and families to develop a care plan which is reviewed and updated with consumers and families on a regular basis. Personal Care Assistants are supervised and evaluated by the SEMCIL nursing staff.

HOME HEALTH CARE SERVICES

SEMCIL's United Home Healthcare Choices, Inc. (UHHC), is a Class A licensed and a Medicare certified home health care provider. UHHC offers a full range of skilled services provided by qualified Registered Nurses, License Practical Nurses, Licensed Therapists, Medical Social Workers and Home Health Aides. UHHC accepts payment from Medicare, Medicaid, county waivers, private pay, and private insurance.

UHHC services are targeted toward achievement and maintenance of an independent lifestyle and incorporates the services available through SEMCIL, Inc. All nursing assessments are conducted by Registered Nurses with expertise in community living and home health. Individual programs are planned with consumers and families to assure that care meets the specific need in a quality, comprehensive and cost-effective manner.

United Home Healthcare Choice Inc.'s services are available 24 hours per day to anyone in need of home health care regardless of payer source.
