About Transition Services
Transition services are offered to individuals 14 to 22 years of age.
Transition staff assist students and young adults to gain knowledge
and the skills they need to live independently. Training is provided
at area schools, SEMCIL offices, consumers' homes, or the community.
Transition services are offered in Olmsted, Goodhue, Dodge, Mower,
Rice, Wabasha and may be available in other counties of southeastern
Minnesota as requested.

Students and young adults will gain:
- An understanding of how to access and direct community supports.
- The ability to make informed choices and understand the consequences of those choices.
- An understanding of their rights as a person with a disability and learn self-representation skills.

About Independent Living Assessment
A two-day independent living skills assessment is used to identify areas of individual strengths and areas for skill building. Staff assess an individual's capacity to perform or complete a variety of activities of daily living. The assessment includes:
